I went to the FAA Safety event last night and the speaker (John Nance) was pretty interesting. Some points that he made included:


* you tend to see what you expect to see which means that if you do a checklist 100 times you may fall into the habit of seeing something that isn't there eventually, he put up a slide that said:

A Bird
In The
The Bush

I missed the error in it until it was pointed out
* the famous basketball gorilla clip, people tossing a basketball, count the number of tosses, did you notice the gorilla walk across?
* tunnelizing on something specific like an instrument and missing the big picture
* "the captain has to be right" irt Tenerife
* it takes a lot of weight to counteract the decision you want it to be
* rejecting the suspicion that something is wrong can be a bad thing
